How Do Individuals Receive Help from
GaPS?
A
variety of agencies refer individuals in need to GaPS. Hospitals
and nursing homes, human service centers, senior centers, independent
home care agencies, social services, and banks. Referrals also
come from attorneys, doctors, and nurses, judges, families, and
neighbors. GaPS often serves as a safety net for the elderly and
chronically mentally ill who often need services to remain
independent, but are unable to secure needed services. We
evaluate the individual's situation, consulting with family, friends,
physicians, social service agencies, banks, attorneys and other
concerned individuals, and coordinate needed residential, medical, and
financial assistance. |
